Between 2001 and 2003, more than 16,900 individuals were admitted to North Carolina drug rehabs reporting cocaine abuse as their primary drug-related issue. Cocaine typically arrives in North Carolina from California, Texas and Arizona. This cocaine primarily originates in Mexico. Powder cocaine is moved into the state via personal or rental motor vehicles. North Carolina is the final stop for some of the cocaine, but some is transported on to other Eastern states like Virginia, West Virginia, Pennsylvania and New York. Much of the cocaine that remains in the state is delivered to crack dealers who will convert the powder into crack and sell it in mainly poorer parts of metropolitan areas. In 2006, almost 556 kilos of cocaine were confiscated during federal drug seizures in North Carolina.
The number of individuals entering North Carolina drug rehab from 2001 to 2003 showed a slow but steady increase. There were 855 admissions in 2001, 889 admissions in 2002 and 957 admissions in 2003. While heroin is not the most threatening drug-related issue in the state, it is available in more populated parts of the state. Mexican drug trafficking organizations smuggle Mexican black and brown tar varieties of heroin into the state from the Southwest region of the country by way of personal and commercial motor vehicles and use of various mail services. Other drug trafficking groups including other Hispanic groups, Asian and African American organizations smuggle heroin produced in South America, Southeast Asia and Southwest Asia to Miami, New Jersey, New York and Philadelphia. The heroin is further moved into North Carolina using personal automobiles and commercial land and air passenger services. Over 3 kilos of heroin were seized in the state in 2004.

Approximately 700 individuals were reportedly admitted to North Carolina drug rehab centers for methamphetamine/amphetamine abuse from 2001-2003. Most of the methamphetamine available in the state is in the form of ice and is produced in Mexico. From Mexico it is smuggled into the Border States or areas of Georgia and then moved to destinations throughout the country, including North Carolina. Just as with their cocaine, these drug traffickers use commercial and personal motor vehicles and various mail services to deliver the ice to North Carolina. Methamphetamine is manufactured within the state as well. Recent laws aimed at controlling the sale of drugs and chemicals needed to make methamphetamine have apparently had some positive effect on the methamphetamine situation in the state. Meth lab seizures dropped from 322 in 2005 to 189 in 2006. Although this still indicates a significant problem in the state, it is does indicate a favorable drop in the amount of locally produced methamphetamine. Roughly 17 kilos of methamphetamine were impounded in federal drug seizures in the state in 2006.
Reportedly the primary drug of abuse for over 17,000 entrants to North Carolina drug rehab facilities, marijuana is possibly the most dominant illicit drug in the state. Although the Canadian variety of marijuana that is smuggled into the state by Vietnamese trafficking groups is preferred in many parts of the state, Mexican-grown marijuana continues to be the most available variety of marijuana in the state. Marijuana reaches North Carolina in all makes of motor vehicles, both personal and commercial. Marijuana is cultivated within the state, as well. Growers are moving from open air crops to indoor grows using aquiculture techniques that apparently yield a more potent product. In 2003, 848 open air parcels and 32,793 plants grown on open air parcels were destroyed in the state. In the state that same year, indoor grows seized numbered 1,490 and indoor grown plants destroyed totaled 34,283.
